If you are taking multiple medications, a specialty medication, or have a chronic condition, Simple Care Medical Services can help. Medication can help keep you healthy and active, but if not taken properly, it can have a serious impact on your health and well-being.
Confidential appointments run 20 to 60 minutes, depending on the number of medications you are taking. During the visit, your Simple Care Medical Services pharmacist will inspect your medication profile and review all prescriptions, over-the-counter drugs (OTCs), herbal supplements, and samples for appropriateness of therapy. They will discuss therapy- and medication-related issues or problems and answer any questions you have about your medications and care plan.
Upon completion of the visit, you will receive a medication list and personalized care plans detailing the just-completed visit with the pharmacist. The plans include times and dates for follow-up visits. Your Simple Care Medical Services pharmacist will also update your doctor, coordinate further care, and relay medication or plan recommendations.

Not all patients require home visits. You can talk to your Simple Care Medical Services pharmacist any time issues or questions arise. Follow-up visits, if needed, will occur at the dates and times set up during the initial visit. Follow-up visits tend to be shorter in length.
The Simple Care Medical Services clinical pharmacist may make observations and recommendations to your physicians about the effectiveness of your current medications. However, all recommendations for changes in medications will be discussed with you before being sent to your physician for final approval. Ultimately, our programs are designed to ensure that you are getting the best results from your medications
